Health Insurance in Oregon

Oregon expanded Medicaid early in 2014 and has one of the more developed individual insurance markets in the country. For 2027 coverage, Oregon no longer uses HealthCare.gov. The state now runs its own marketplace, Explore Health, at ExploreHealthOR.gov. Four insurers sell 2027 plans there after Providence and PacificSource left the individual market. Oregon's Oregon Health Plan (Medicaid) covers over one million residents, and the state has been a leader in Medicaid innovation through coordinated care organizations.

Medicaid in Oregon

Oregon expanded Medicaid in 2014 under the Oregon Health Plan (OHP). The program covers adults aged 19 to 64 with household incomes up to 138% FPL (about $22,025 for a single person for 2027 coverage). Over one million Oregonians are enrolled in OHP, making it one of the largest Medicaid programs relative to state population in the country.

Oregon delivers Medicaid through Coordinated Care Organizations (CCOs), which are regional entities that integrate physical health, behavioral health, and dental care. This model has been nationally recognized as an innovation in Medicaid delivery. CCOs are accountable for outcomes and spending, not just service volume.

Oregon's uninsured rate is approximately 5.5%, with about 230,000 residents lacking coverage. The state has made significant progress reducing its uninsured population since ACA implementation.

Explore Health: Oregon's new marketplace

Starting November 1, 2026, Oregonians shop for marketplace coverage and apply for financial help at ExploreHealthOR.gov, not HealthCare.gov. Open Enrollment for 2027 coverage runs from November 1, 2026, through January 15, 2027. If you enroll by December 31, 2026, your coverage starts January 1, 2027.

If you had a HealthCare.gov plan in 2026, sign in to Explore Health to update your application and review your 2027 options. People whose insurer is leaving have been matched with a plan from another company for automatic re-enrollment, but you will usually do better by comparing plans yourself. Explore Health's phone line is 855-268-3767.

What plans cost in 2027

Oregon's Division of Financial Regulation approved individual market rate increases averaging 21.6% for 2027. The state reinsurance program lowered rates by an average of 10.7%, so the increases would have been larger without it.

KFF puts Oregon's average benchmark Silver premium for a 40-year-old at $519 a month for 2027, down from $543 in 2026. Average rate increases and benchmark prices can move in different directions when the lineup of plans changes, so look at the actual prices in your county. The Portland metro area has the most choices, while rural eastern and southern Oregon counties have fewer.

Oregon had approximately 160,000 marketplace enrollees in 2025. The enhanced subsidies expired after 2025 and were not extended, so for 2027 there is no premium tax credit above 400% FPL (about $86,560 for a household of two). Oregon's Division of Financial Regulation reviews all rate filings and can reject unreasonable increases.

Carriers in Oregon (2027)

Four insurers offer individual plans through Explore Health for 2027. Providence Health Plan and PacificSource no longer sell individual coverage in Oregon. Every county has at least two insurers, and BridgeSpan and Regence are available statewide:

  • Kaiser Permanenteoffers plans in the Portland metro and parts of the Willamette Valley, a smaller area than the other three insurers. Kaiser operates its own hospitals and clinics in an integrated model. If you already use Kaiser doctors, their marketplace plans are a natural fit. Kaiser's closed network means you generally must use Kaiser facilities.
  • Moda Health is an Oregon-based carrier offering plans in 28 counties for 2027. Moda is not available in Crook, Deschutes, Klamath, Lake, Marion, Polk, Umatilla, or Union counties.
  • Regence BlueCross BlueShield of Oregon offers plans in every county.
  • BridgeSpan Health (a Cambia Health Solutions subsidiary, like Regence) also offers plans in every county.

For 2027, seven counties have four insurers, 23 have three, and six have two. If you had a Providence or PacificSource plan in 2026, you need a new insurer for 2027. Check that your doctors are in the new plan's network before you enroll.

Key regions

Portland metro

Multnomah, Washington, and Clackamas counties have the most competitive marketplace in the state. OHSU (Oregon Health & Science University), Providence, Legacy Health, and Kaiser Permanente are the major hospital systems. All four 2027 insurers sell plans in Multnomah, Washington, and Clackamas counties. This region accounts for the majority of Oregon's marketplace enrollment.

Willamette Valley (Salem, Eugene, Corvallis)

Marion County (Salem), Lane County (Eugene), and Benton County (Corvallis) are served by several carriers. Salem Health, PeaceHealth (Eugene), and Samaritan Health Services (Corvallis) are key providers. Moda does not sell 2027 plans in Marion or Polk counties, and Kaiser covers only part of Lane and Benton counties.

Central Oregon (Bend, Redmond)

Deschutes County has seen rapid population growth. St. Charles Health System is the major provider. PacificSource, a longtime carrier here, left the individual market for 2027, and Moda does not sell plans in Deschutes or Crook counties. That leaves BridgeSpan and Regence. Specialist access sometimes requires travel to Portland.

Rural eastern and southern Oregon

Counties like Harney, Malheur, Lake, and Klamath have limited healthcare infrastructure and two or three marketplace carriers for 2027. Critical access hospitals serve many of these communities. Premiums are higher, and travel for specialty care is common. Sky Lakes Medical Center (Klamath Falls) and Asante (Medford/Ashland area) are the key systems in southern Oregon.

Oregon-specific things to know

No catastrophic plans for 2027. Explore Health says Oregon insurers are not offering catastrophic plans for 2027. Every insurer does offer Oregon Standard Bronze, Silver, and Gold plans, which include three office visits at $5 each before the deductible.

Rate review provides some protection.Oregon's Division of Financial Regulation actively reviews marketplace rate filings and can reject unreasonable increases. This provides a layer of consumer protection that is not present in all states. Rate decisions are made public, so you can see how much each carrier requested versus what was approved.

Oregon Health Plan is generous. If you qualify for OHP (income under 138% FPL), the benefits are comprehensive with no premiums and minimal cost-sharing. Oregon's CCO model means your care is coordinated through a local organization. Check OHP eligibility before shopping on the marketplace.

Free enrollment assistance. Oregon has a robust network of community-based enrollment assisters and certified navigators. Explore Health has a local help finder on its site, and community health centers also provide free help with applications.
